23-24 Registration Form For Confirmed Scholars (Only)

Appointments will be scheduled to complete registration before School Year 2023-24 begins. Once your paperwork is complete, please call our admissions department at (910)-814-9001 Ext. 1160 and you will be instructed to email your paperwork and make an appointment for in-school registration.

Below are items that must be included in registration when you arrive for your in-person registration:

Proof of Residency
Copy of Current Immunization Record
Birth Certificate
Copy of Driver License
North Carolina Health Assessment Transmittal Form
