Required Documents for For Confirmed Scholars (Only)

Appointments will be scheduled to complete registration before School Year begins. Once your paperwork is complete, please call our admissions department at (910)-814-9001  Ext. 1160 and you will be instructed make an appointment for in-school registration.

Below are items that must be included in registration when you arrive for your in-person registration:

• Proof of Residency

• Copy of Current Immunization Record

• Birth Certificate

• Copy of Driver's License

• North Carolina Health Assessment Transmittal Form
